Cannulae use versus needles

Dear Doctor, reading your blog/posts I noticed that you use cannulae to put the filler "in".
Let me see if I understand why- it is to not risk to hit a blood vessel and inject the filler in it, right?

a) What are the risks of the filler getting in the vessel? its traveling through the bloodstream and eventually cause migrations, necrosis, etc.?

b) If we have naturally hyaluronic acid in our body, why having it in our bloodstream is dangerous?

c) How do docs who use needles, not cannulae "know" that they are not hitting a blood vessel?

YES, BUT ALSO BECAUSE IT IS A SMOOTHER LOOKING RESULT AND LIMITS BRUISING AND IS SAFER AROUND THE EYES.

REALLY NOT MIGRATION, JUST NECROSIS. OBVIOUSLY, NECROSIS IS A BIG THING BUT PRETTY DARN RARE. I AM NOT TRYING TO BE A FEAR-MONGER.

b) If we have naturally hyaluronic acid in our body, why having it in our bloodstream is dangerous?
NO HAVING IT IN THE BLOODSTREAM IS FINE I GUESS. IT IS THE ANSWER TO POINT A ABOVE.

c) How do docs who use needles, not cannulae "know" that they are not hitting a blood vessel?
THEY SHOULD PULL BACK ON THE HUB AND MAKE SURE THEY DON'T SEE A FLASH OF BLOOD. THAT IS THE EASIEST WAY BUT STILL NO ABSOLUTE GUARANTEE.

Does the same apply to the cheeks? That is, are there less safety concerns when the filler is put in the malar area versus the risks associated with needle versus cannula for the area around the eyes?

Is necrosis more or less likely in the injection in cheek areas?

I want to be careful how I answer that question. I think the cheeks are pretty safe. In general, I think needle injections in the face are pretty darn safe in experienced hands. I rather leave it at that.
