malar edema

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

malar edema

can anything be done about malar edema. My doctor said that I have water retention and not fat. He said nothing can be done. However I came upon a few articles where excess muscle/skin can be excised and tension placed upon the area effectively effacing the festoon.

what is your opinion on this dr.lam?

I actually graded these types of malar bags in my book, Complementary Fat Grafting. Low grade malar bags have just recurrent swelling but no abnormal skin. A high grade malar bag has both recurrent swelling and abnormal overlying skin. With low grade malar bags that really only swell and look abnormal in the morning, it is not a good idea to cut them out. you will probably just leave a scar but not solve the problem. there is not a great answer for this type of bag. for an advanced type of bag with bad overlying skin, removal of it is probably the best bet. i have attached a result of a gentleman whose bags i had to remove.
