i use either sedation (iv) or GA. straight local is not great even if others do it. a huge reason is that people that do fat under straight local use a lot of tumescence (which i do for facelifts). hearing from docs who do tumescence, i truly think the reason that they have to do fat grafting so many times to get a good result is that the tumescence (extra fluid to cut down on discomfort) compromises the fat longevity. therefore, with a full surgery center on site, i prefer at least some mild intravenous sedation. My anesthesia is very safe because the procedure is relatively short (a couple of hours) plus this is not body contouring which exposes a person to a lot of volume shifting. All the problems you have heard about in the press is related to body plastic surgeons doing 7 to 8 hour body plastic cases. Check it out.
best,

yes, my longest procedure is 4.5 hours when i do EVERYTHING. but even then there are no substantial fluid shifts.
