Eye Surgery Questions

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

Eye Surgery Questions

Hi Dr. Lam,

I've been looking into getting asian eyelid surgery done and doing alot of research on the matter. I read that Asian skin is more sensitive to scarring than Caucasian skin and surgery often leaves behind a raised red welt. Some doctors would treat the site of the scar with laser to decrease blood flow to the scar and smooth the scar. This is the first time I read about it. Do you find it necessary to do laser on patients that has asian eyelid surgery done to get rid of scars?

Another question I have, is my mom is wanting surgery done to get rid of the bags under her eyes. There is a procedure that some doctors used where the incision isn't made outside the eye but inside the lower lid through a lining called the conjunctiva. The tissues heal very quickly and stitches is not needed. Have you ever heard of such procedure and do you do that type of procedure in your practice?

First of all, I have seen some redness persist with incisions in Asians but rarely so in the upper eyelid. That should be camouflageable with makeup for the most part. Although scarring is possible, I don't think it is common, as I have not seen it before, at least not on the upper eyelid. Most lasers are unsafe on the eyelid. I use lasers really outside of the orbit (bony eye bone).

Yes, I do not cut in fact never cut the lower eyelid. I did my own mom that way, i.e., from inside. However, that technique is simply not enough and oftentimes not even necessary. Most people suffer from hollowing. In my mom's case, I removed the eyebag and then added lost fat around the rim all without a skin incision. Please watch my video in my video logs section on safe lower eyelid surgery and also my video log on balloons, shorelines, etc. Those two videos will help you understand my philosophy better until we meet.

thanks. she looks even better now. she is 8 months out. fat grafting really starts to look good 8 months out especially about 2 years out.
