Micro Droplets?

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

Micro Droplets?

Hi Dr Lam-

Appreciate the time you take to answer our questions...

You imply that your "micro droplet" approach to fat grafting is somehow different/superior than what most doctors do. I had assummed (I guess incorrectly) that all doctors would inject fat that was "broken down" to the same physical size and that you would all be using very fine needles.

Can you explain how your technique differs? If everyone is using fine needles wouldn't everyone be creating "micro droplets"? Is the only difference that you try to lay down a very thin layer of fat at each pass (like a sheet of paper) while some others may inject bigger amounts per injection (shaped more like globs)? And even if you do lay down fat in a much thinner layer per injection don't you still have multiple layers just stacked on top of each other leaving you the same basic result as having injected that mass all at once?

you should do some archive searches on this forum because i have answered this question in detail. however, i will get to the heart of your question. by multiple minor passes and by making these passes through different tissue depths, you can literally layer the little droplets into the tissue so that most of the droplets stay as droplets. no needles are used but cannulas so the droplets are held more securely in the tissue planes than with a needle that lacerates tissues. plus, i do not use huge volumes of fat. some of my colleagues use a lot of fat which ultimately becomes fat stacked on fat, leading either to random uncontrolled absorption or an overfilled face.

We might add to this explanation inexplicable giftedness. If we could ask Tiger Woods (or Renoir) why he's so great, better than most, how could he explain...more practice? Technique? It has to be something else, more esoteric. Your patients (I for one) might simply say you're able to do this work better than others.

Boy, that was a nice comment right before I go into the OR this morning. I don't think I am special but I certainly love my job more than most people can imagine. I have a strong aesthetic eye and thank God I am getting great results!
