This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

With the macs lift from years ago, the skin was pulled up too much in a vertical way, resulting in too much skin under the eyes, its becomming worse and worse, since i had no hollows to begin with, it looks puffy under the eyes, like the fat and wrinkles are starting directly under the lashes. It makes me very tired and is troubeling my vision. Is an undereyelid surgery needed to correct this problem, upper eyes can de done at the same time, together with fat transfer to the face.
i personally do not cut on the lower eyelids because i do not like taking the risk to change eyelid shape that way. therefore, i am not your surgeon to do corrective lower eyelid surgery. you need to ask another doctor who does skin-muscle flaps, i.e., skin based lower eyelid surgeries. obviously, that doctor would have to see you in person to decide that. but as i stated i do not perform that kind of lower eyelid procedure. best, sml

Hi dr Lam, I have sent you the before pictures, and recent pictures. Also one from a few years ago
Maybe you can have any idea if the graft would work well. I am afraid i may never have not enough skin to make my face look natural as i did before. So much skin is taken away. Note that in the recent pictures there is a bit of juvederm injected under the brows, near the eyes on the sides, in the nose tip, under the nose , in the lips, trying to even them out a bit. and on the left side of my face , still more hollowed then the other due to the uneven pull on the skin. Also a bit in three areas of the upper cheeck, in fact the area under the eyes, it looks pretty bad, i am constantly looking at a tickness that should be much lower. All this because the skin was lifted way too high.
These are only tiny tiny bits and gone in two months, mostly three. Sorry about my complaints but maybey it is possible to redo the lift, redrape the skin and at the same time the fat graft in the right places,
Hope there is some solution Best regards, Martine.
martine, i have carefully reviewed your before and after photos, and fortunately i do not see anything as devastating as you are pointing out. now, that is not meant to discount how you feel about what happened but i am being honest when i look at your photos. i do not think you look terrible but i do see a profound sadness in your eyes that is making you look worse than you should. now, obviously, seeing you in person will give me the best answer. however, you know my concerns and they are real ones. you need to lift yourself out of this blame-game situation on this lift leading to all your woes in your life. doing so, will only stop you from living. when i see your photos i do not see anything that i am dreadfully concerned about. please free yourself from your own prison. no one else can do that. no surgeon. no friend. no counselor. only you have the keys to shed you of your past. a fat grafting is not a solution to your emotional situation. will it help your face? of course, yes. i can see the benefits. but you probably will not after your procedure until you get out of the dense fog of blaming your past. YOUR PAST IS GONE. IT WILL NEVER COME BACK. please help yourself by moving forward. until that happens, i cannot help you. when you are free from your own hell, i can help you achieve your aesthetic goals, which i look forward to do. much love and respect, sml

Thanks dr Lam to answer me that quick. I know what you are telling me is all right , I know this and I know I have to do that. I am working on it and some days it is more difficult then others. I am mad at myself why i did this facelift and not looked for more suitable options. I blame myself for it and it has messed up my life.
I know i have to find peace with everything before i do any surgery.
I am already glad to hear that fat grafting would be a benefit.
Do you think it is nessasary to redo the facelift ? or only the fat grafting ?
Is it possible to be a bit more specific on that. I am pleased that you want to help me in the future.
let your anger go. it is the first step. when you have forgiven yourself you will be ready.
i do not believe (based on the photos) that you need to redo your facelift. a fat transfer will soften your features and return you principally to where you were. to be honest, you needed a fat transfer 3 years ago, and i hope to make you look way better than even those photos.
first, offer yourself the deepest forgiveness so that you can close your chapter in your life and move forward. we are here on this earth for a short time. don't make your short time a sad one. we have many things we can be grateful for. start with those. write them out on a sheet of paper. best, sml

One more question, If you look at the recent pictures, do you think I need upper eyelid surgery before the fat graft, it seems i have too much skin there and can not put on make up any more, the fillers under the eyebrows are making my eyes only more deeply set and putting on mascara makes iy worse as then the eyelashes are toucing the skin. Thanks again so much for your opinion. Martine.
