alarplasty

Greetings Dr. Lam,
I'm interested in getting nostrils reduction and a tip plasty. However, I'm worried about visible scarring from the nostril reduction. I've read from other messages that you are somewhat skeptical about performing alarplasty. But this is the recommended procedure for me as I have flaring nostrils that you could see a mile away. I'll be having a phone consult with you in a week and my photos will show you just that. How will you approach this procedure?

Let me review your photos and explain this more in detail during the consultation. In general, you are right. There is a much longer recovery involved with an alarplasty. If you need it, you need it. I´ll make those discussions with you. Bottom line is that I perform a Sheen flap in which the entire ala is not cut through so that I can preserve the natural curvature of the ala. In addition, you may need to have dermabrasion a few months out to make it look as perfect as possible. I perform that in about 25% of the cases.
