these are my thoughts on fat grafting that may not pertain to your case since your surgeon and i did the work: 1. fat grafting does not perfectly fix tear troughs unlike fillers but they allow me the ability to truly get a very good result at less expense when working on the entire face. that is why i never just do tear troughs. if that is the case, i would use fillers. 2. at one week you are way too early to read anything. unfortunately, i believe that fat needs a year to mature to see how things go over time to read the result. sometimes it can dip and then improve again but probably in your case not toward perfection. 3. i would not add fillers at one week as you might compromise the blood supply of the fat. honestly, i would wait several months at the very minimum to see how things are going. 4. if you were to do fillers this year i would consider restylane which would allow for fat changes. however, i personally do not think you can get as good a result or fill as well under the eyes with restylane as you can with artefill, which is permanent like fat but may take 2 rounds to get a near perfect (but not perfect) result. it is something i am very good at doing using microcannulas to minimize bruising and to attain excellent results. in summary i would wait 9 months and if things are not improving i could help you with artefill to make your result better if you needed it. you would need a collagen skin test first to make sure you are not allergic to the product. best, s.
