help to restore firmness?

Dear Dr. Lam, I am learning so much from your videos and website. Thank you for treating patients with such respect. I am 57. I had fat grafting a few years to help fill out my slim face, and that has been a plus. However, the lower part of my face is losing firmness and starting to get a little jowly and saggy. In hopes of postponing surgery, I purchased a microcurrent facial device which says it will stimulate collagen, and improve sagging. What is your opinion of these devices? First I want to make sure I'm not doing any harm (it is FDA approved-but I'd like your opinion) and second I wonder if it will really be of benefit. Also, do you recommend further fat grafting in the lower half of the face to help restore firmness?

honestly, i do not know the answer to your question whether those techniques work or not. to tighten sagging I prefer the Accent XL but to be honest that does not truly fix things, just makes them better by a bit. i do not think fat transfer will tighten things to be honest. it is intended to improve the volume but not the tightness. a facelift can help but it is really for muscle sagging not skin looseness per se. probably some aggressive skin resurfacing if you needed it might be the better solution like plasma skin resurfacing, CO2 laser, etc.
