Orbital Fat grafting questions

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

Orbital Fat grafting questions

Hello Dr. Lam
I'm interested in getting fat transfer in order to make my eyes appear less hollow and less deep set on . I have the following :
eyelid ptosis
upper and lower eyelid hollowness
sub brow hollowness

(you can clearly make out the shape of my eyeball therefore my crease is really the superior sulcus of the orbital rim).

It really ages me quite a lot. I'm a healthy female who hasn't had any weight fluctuations so I'm not sure why I have such a gaunt look about me... i seem to have this look as long as i remember (i'm currently in my mid 20's and get constant questions from people asking me if I'm ok, since I look tired, sad, depressed etc).

Should I get ptosis repair before getting fat grafting ? Or will ptosis repair fix my gaunt look?
Will fat grafting allow for me to have a crease?
I would like this operation to be permanent is that feasible?
I heard that pearl & strip fat grafting is the way to go while others claim that small micro fat grafting is better in terms of not being metabolized. -- My concern is what is best for my case as well as permanent results.

Should I get ptosis repair before getting fat grafting ? HARD TO SAY. I COULD DO FAT TRANSFER AFTER PTOSIS SO LONG AS THE OTHER DOC ALLOWS ME TO PUT FAT IN THE SENSITIVE AREA OF THE UPPER EYELID. IF NOT, I SHOULD GO FIRST WITH FAT. Or will ptosis repair fix my gaunt look? NO, NOT AT ALL.
Will fat grafting allow for me to have a crease? YES. BUT AS YOU KNOW IN YOUR AGE GROUP YOU HAVE TO BE VERY COGNIZANT THAT YOU CANNOT GAIN WEIGHT AS YOU AGE OR THE FAT WILL NOT LOOK GOOD FOR YOU. I WOULD HONESTLY SUGGEST TEMPORARY FILLERS BUILT UP SEVERAL ROUNDS AND WHICH IN MANY CASES CAN LAST MANY YEARS (NOT IN EVERYONE) AND ALMOST BE SEMI-PERMANENT.
