Jaw Reduction + Cheek Fat Grafting at the same time?

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

Jaw Reduction + Cheek Fat Grafting at the same time?

Hi Dr. Lam,

You are probably familiar with the procedure they do in Korea called "Jaw Reduction" or "V-line surgery" in which they shave off the angle of the jawline and reduce the chin in frontal view. I was wondering, can this procedure be done at the same time with cheek fat grafting? I plan on doing jaw reduction in Korea, and I also want to do fat grafting at the same time. However, after the jaw surgery, I have to wear a bandage wrapped around my head and there will be tremendous swelling on my lower cheeks/chin area. I am worried that this will affect the fat graft. What do you think?

P.S. In the Jaw Reduction surgery, some muscles will also be resected, which is a further concern with it affecting cheek fat transfer result.

the problem is that i do not do that procedure so i cannot directly answer your question. however, i do facelifts with fat grafts all the time, working with my facelift in exactly the area you are talking about and my faces are wrapped for one night relatively tightly and then thereafter wrapped snugly but not too tightly. i would assume you would be ok but make sure you speak with someone about this who would be doing your procedure so you get a more accurate response.

Thank you for the response, Dr. Lam.

I did end up having the procedures and afterwards, a bandage was wrapped VERY tightly around my head for 24 hours. I had fat grafting in the hollows under my cheekbone, so some of the areas that had fat grafted received pressure from the bandages for 24 hours.

Just to clarify - this kind of pressure on newly grafted fat won't hurt the fat or its survival? I'm really terrified about receiving lumps. It sounds like your facelift patients receive the same kind of pressure on their fat grafts and it does not cause a problem?

I had this procedure done in Korea and I am not Korean so I cannot communicate too clearly with the surgeon there. I really appreciate your knowledge/experience in this matter.
