This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)
Attached my old and new photos for comparision. Please advise which parts of the face you would recommend for fat transfer to restore my youthful look, as seen in two of my old photos. What I have in mind is the lower eyelid, cheek, buccal areas, nasolabial and marionnette lines. Please commnet.

we all age all over the place so the trick of beautiful, balanced fat grafting is that there is a little bit every where. where?: upper eyelid/brow, temple, lower eyelid, tear trough, outer eye can thus, anterior cheek, lateral cheek, buccal, subzygomatic/backfill, canine, anterior chin, and pre jowl. that is, the whole face. the amount and proportion is what i would differ on in your case which is hard to explain in a forum response.

Thank you for your reply. But I am still doubtful if I should do fat injection or re-positioning to correct my tear trough depression.
For your information, I have done fat transfer twice on my upper eyelid + tear trough + nasolabial grooves areas. Once on my cheek only.
My lower eyelid still looks puffy with depression still obvious. No fat injection has been done on the areas surrounding the lower eyelid.
I have tried to stretch the fat injected lower eyelid areas slightly outwards, it seems that the puffiness has gone and I look so much youthful. Hence I am confused if fat re-positioning would be a better option instead of fat injection.
i personally do not perform fat repositioning because in most people that need fat in the lower eyelid there is not enough fat to reposition. it is in my opinion highly inaccurate way to fix things. If you want precision in one area I would go with a filler like Restylane (temporary) or Artefill (permanent). hope that helps. s.

Please advise if it is appropriate to add fat in the arrowed area, the gap in between our outer nose and our inner eyelid, as I notice my eyes look smaller due to fat depletion in the arrowed area.

sorry, in short no. fat cannot be squeezed in there and look good. s.
