I know you do not like to do alar base scar revisions. But I was wondering if my scar is a mixture of half minimally hypertropic (small bumps where skin pulled while healing) and some indentation to it, would you consider silicone gel on some and dermabrasion on the rest? Last dermabrasion it seemed my scar was darker and more hollow with my surgeon so he revised it. The revised version is better however some parts are as described above and I wonder if he were a little aggressive in the dermabrasion. Also he seems to discourage laser therapy to this area although I read a couple of studies were women actually benefited from a depressed alar scar.
obviously, i cannot really answer your question until i see you in person. however, in general, silicone gel really does not affect any bumpiness too much but it is worth a try. dermabrasion can also help but hopefully you are not a person of color, as there is some risk of lightening the skin in the area. yeah, laser in general won't really help too much in my opinion. alar base scars are never easy to fix. just remember, it will always be two shades less than ideal. don't push it to try to get perfection because it is unattainable and may risk further scarring. those are my opinions. best, s.

If I send you a picture would you be able to tell me if dermabrasion and silicone would help. Silicone for indented areas and dermabrasion for the bumpy areas
you are welcome to do it but most likely it won't really help me too much. these scars simply are hard to really improve much in my opinion. s.

dermabrasion is typically the best answer if there is room for improvement but that is such a subtle thing that photos really are hard to read if that would be justifiable. i would ask your surgeon who is taking care of you about the room for further improvement. best, s.
