tca cross on second visit

This section of the forum is dedicated to discussions on managing, treating, and correcting acne scarring. (Of note, Dr. Lam is focused principally on acne scarring rather than on managing the ongoing condition of acne, which a dermatologist would be better equipped to address.)

Hi Dr. Lam. I am a former patient. I came in about three years ago regarding my acne scarring. I had a consultation and then procedure with you using the TCA cross method. You said that I required only one to two treatments. Being short on money at the time, I went with the one treatment. Lately I feel like I could go for a second round and that my skin could use some improvement. At the time you denied me silicon microdroplet injections because you said that my skin was not severe enough to warrant it. However I would like you to take another look as it has been bothering me. Also, I'm not sure if I saw results with the cross treatment or not. It was too long ago and I don't remember how my skin looked before. But these are my questions;
1) Since it has been three years since my first (and only) tca cross treatment with you, will that lessen the significance and potential outcome of the second cross? I know that these treatments are best done at intervals.
2)I forget, how long to wait in between tca cross treatments?
3) Three years ago, I paid a hefty price tag for 10 minutes of your work (I'm not downplaying your hard work and artistic abilities.) Has that price changed since then? If not, is there some sort of payment plan that could be worked out?
4) Would you reconsider the silicon in my particular case? I think you may have my picture on file. And trust me, when you saw me in your office the lighting wasn't the harsh lighting that really brings the scars out.

I know that these treatments are best done at intervals. the collagen building from the TCA should not go away. although i have not had anyone this long a time between sessions i do not think you are starting from scratch.
2)I forget, how long to wait in between tca cross treatments? if the scarring is severe, then once a month. if it is mild then i spread it out a minimum of every 3 months.

If not, is there some sort of payment plan that could be worked out? no, same price. we have care credit for you. as far as your question on silicone goes, i am happy to revisit it. i have become more aggressive with silicone in areas that i traditionally only did TCA. i would have to see you again though to make sure that you are a candidate. reviewing your photos is not a good idea because it is so hard to see what kind of scarring you have in a photo unless i see you in person.

thanks for the response. I know this may not be your area of expertise or what you deal in but could you briefly explain the care credit system? I just bought a home and funds are tight. Monthly payments would suit me the best at this moment in time.

i don't know how care credit works exactly. go to care credit.com or google it. you pay essentially a percentage on the loan and then pay it back that way. i would wait until you can afford the few hundred for the procedure before doing the loan. i don't want you to be suffering for this. i think it would be better that way. :)

hi dr. Lam. I recently came in from utah and you treated my scars with silicon. I forgot to ask you if you would advise a second treatment or is the first treatment alone sufficient? it's four days post procedure and I have no swelling. I am pretty happy with the results but think they could be improved still. Do you think I could use a second go around? if so, how far away in advance would you recommend i schedule the appointment? thanks.

remember not to get too excited with 4 day results. i personally would wait at least 3 months before i consider doing it again to let some of the collagen remodeling to occur. typically with silicone it takes 2 to 3 rounds to get the area improved.
