Depressed scar on nose

This section of the forum is dedicated to discussions addressing linear scars, depressed scars, keloid scars, hypertrophic scars, and other facial reconstructive topics. (Of note, Dr. Lam no longer accepts medical insurance so he rarely manages cancer reconstruction or trauma anymore.)

Depressed scar on nose

Dear Dr. Lam,

About two months ago I injured my nose and while I'm no stranger to scars and understand the healing process take time and lots of patience i was wondering if my window to help make the depression of the scar dissipate has ended (whether it be through silicone strips or whatever other less invasive methods). I also understand that you can't dispense any medical advise directly, however maybe you can put to rest some methods that are less effective. Although the scar as it is minuscule and because of that I wouldn't consider surgery, however i work as a promotional model and although I've lost work in the last two months I'm more concerned about the long term effects I'll face in this line of work. With the scar on the focal point of my face, I'm very hopeful with any light you may be able to shed on the situation while at the same time very practical and realistic. Thank you for your time.

at this point if it does not get enlarged, red or hard, then tincture of time (that is, waiting) is the best method to see how things progress. little else will help. if it becomes thick, red, or angry, then laser and injectables like 5FU can modulate the recovery in a favorable way.

Dr. Lam,
It's been about seven months since I've injured my nose and although the overall color has improved and help camoflauge the scar the indention hasn't filled in. I really wanted to avoid surgery at whatever cost so I've leaned toward other treatments like silicone micro droplets but every plastic surgeon ive spoken to tells me they wouldn't recommend silicone. I understand it is permanent and lasts up to 20 years, but what exactly happens at the end of that 20 years?

i have been only doing it for less than 10 years without any long-term problems if done with micro droplets and spaced slowly over time. however, physicians have done it for 45 years without problems if the silicone is pure grade silicone and only small amounts are placed into discreet areas using the sequential micro droplet technique. conversely, the risk of injecting high volumes in non-microdroplet technique without spacing out sessions is that the silicone drops can coalesce and then migrate.
