TCA or Silicone

This section of the forum is dedicated to discussions on managing, treating, and correcting acne scarring. (Of note, Dr. Lam is focused principally on acne scarring rather than on managing the ongoing condition of acne, which a dermatologist would be better equipped to address.)

TCA or Silicone

Hi Dr.

I was wondering if you could tell me if silicone injections would help me.... Im not too interested in TCA cross since I hate to wait for the discoloration to go away which could take over few weeks to completely recover with 100% cross and im indian so not sure if it would be appropriate....

I have about 12-13 deeppitted scars (and small shallow pits around cheeks) - since they are more angular and wide i would say its boxscar? would you agree? Secondly, i live in australia so i will need to fly all the way to US to get treatment done - can i get two MDS done within 1 month while Im there??? I have tried needling - with no luck!
I also have small bumps all over my skin - is there a way i could take care of them - however, my biggest problem is the deep pits on my skin - would silicone be safe for me to use and how much improvement is likely with this method?

some of the bigger holes may be a bit helped with silicone but not much. i agree TCA is too much of a risk in your skin type. honestly, i would discourage you coming all the way from Australia to be treated. i feel that you will most likely be disappointed in the silicone given what i am seeing. so sorry.

Thanks Dr. Lam - Going through other posts/information i thought silicone would be best for larger pitted type scars which is what I thought.... Could you explain my kind of scarring i.e. boxscar/icepick??? i dont care so much about the rest of the uneveness/small pits....the only few major pits are bothering me quite a bit - about 7-8 main ones... You dont suppose having 3-4 rounds of silicone on those particular pits would be of much help??? I dont know what else i could do to improve the texture :-((

ok, yes, silicone will probably improve the big ones. however, you have enough small ones that i thought you would be bothered they would not be improved much. :)

Thanks for your reply! I see some hope now - just wanted to ask u some specific questions:

1) Do you perform silicone injections on individual scars or around the area? What I mean is if you inject one pit does it plump up the skin around that pit??
2) Is silicone injections best for deeper pitted scars that becomes normal skin when stretched?? I have about 7-9 pits that are deep and cant be covered with makeup - however, when i stretch the skin - they look normal so im guessing these are boxscars?
3) I know its hard to advise me on this without seeing me in person - but to say generally, are 3-4 rounds of silicone injections to few deep pits like mine enough to make it more even with rest of the skin?? Im just hoping to see improvement within those deep pits to atleast be covered by makeup - not expecting a perfect result. Please advise.

WHEN I INJECT THE SCAR, IT SHOULD STAY IN THE SCAR. I KNOW HOW TO DO IT SO THAT I REALLY DO NOT WORRY ABOUT THE DONUTING EFFECT YOU ARE TALKING ABOUT.
2) Is silicone injections best for deeper pitted scars that becomes normal skin when stretched?? I have about 7-9 pits that are deep and cant be covered with makeup - however, when i stretch the skin - they look normal so im guessing these are boxcars? STRETCHING THE SKIN DOES NOT GIVE ME A GOOD ACCURATE VIEW OF THINGS. I TRULY GO BY WHAT I SEE TO DETERMINE THE ONES THAT WOULD BE BENEFICIAL AND THE ONES THAT I DO NOT BELIEVE SO.

WHEN I SEE SOMETHING IN REAL LIFE I GET A MUCH BETTER READ ON THINGS. I HATE TO MISLEAD ANYONE ON HERE BECAUSE PHOTOS ESPECIALLY WITH SCARRING ARE SO LIMITED. REMEMBER WITH ANY ACNE SCARRING, IT IS HARD TO GET HUGE IMPROVEMENTS. JUST SOME IMPROVEMENTS IS ALL YOU CAN EXPECT ESPECIALLY WHEN YOURS ARE NOT SO DEEP. WHEN SOMEONE HAS CRATERS IN HIS FACE, THEN I CAN WORK ALMOST MIRACLES. I KNOW YOU CONSIDER YOURS DEEP BUT THEY REALLY ARE NOT COMPARED WITH THE ONES I HAVE SEEN.
