can this scar look better.

This section of the forum is dedicated to discussions addressing linear scars, depressed scars, keloid scars, hypertrophic scars, and other facial reconstructive topics. (Of note, Dr. Lam no longer accepts medical insurance so he rarely manages cancer reconstruction or trauma anymore.)

Hi,
15 months ago I was splashed in the face with acid at work resulting in in burn marks on my face. One is particularly big and extends from my temple, down around my outer eyebrow and curving toward my eye and ending on the outer side of my eyelid. To make matters worse, the acid that accumulated in the outer crease of my eyelid burned a hole right through. The hole was sort of the shape of a horizontal elipse and when it finally contracted and closed, it pulled up on the side of my eyelid and kind of fused my eyelid folds together. so the side of my eye is slighltly pulled upwards and the creases are fused/puckerd together. Anyway, I am hoping for a scar revision that can make the corner of my eye look normal and not pinched and pulled up, and I'm hoping some of the burnt skin can be removed and the defect covered by local flaps. I would much prefer a line with zig zags then such a wide swath, but don't know if it's possible. Here is a picture, although it was taken not long after the accident, even before the hold closed up. It looks much the same now excpt my eyelid is pulled up. any suggestions on improving this would be so helpful. It has really affected my self esteem. hmmm. I'm having trouble with the picute. I will try later.

Here is a picture from early on. the scar is the same shape but not red. it's very white and slightly depressed with a rough wrinkly texture.

hi,
1. the scar looks wide. i would not cut it out or you would have a really large scar.
2. it could be dermabraded if the edges are not even or filled with filler if depressed. i would have to see it.
3. for the area that is causing the fused issue of the eyelids, you need to see an oculoplastic surgeon to work on that. i would not go to a "standard" plastic surgeon or even a facial plastic surgeon. you need a specialized eye plastic surgeon (oculoplastic surgeon) to handle the situation.

Thank you so much for your response. I will find an occuloplastic surgeon about my eye. About the other part though, could it be made narrower? not cutting the whole thing out just maybe part of it? ? Also, would laser resurfacing have the same effect as dermabrading to smooth out the edges? there is an obvious border around the scar. thanks again.

1. i would not cut it out even a little bit because the scar is too wide. all you would do is leave now a bad linear scar. not a good idea from what i see. things maybe different if i saw you in person but unlikely.
2. dermabrasion and laser may be similar depending on who does it, what laser (hard to give a rec though) and how it was done. don't even know if you need it. would really have to see your skin up close and personal.
