Facial fat grafting at 25?

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

Facial fat grafting at 25?

I appreciate that this idea may seem a little extreme. However, I am unhappy about the fact that my face has taken on a slightly more angular and hardened look since my teens, and I was wondering if, purely technically speaking, it would be possible to recreate the look of teenage softness with facial fat grafting? I understand that pregnancy and later weight fluctuations can detract from a fat transfer's success, but if one were not to ever have children or fluctuate in weight, would it be possible?

(I am speaking purely hypothetically here; obviously people change their mind about having kids. I just wanted to know if it was possible, purely from a technical standpoint).

yes, theoretically that is possible. however, remember that adding a lot of fat may look good in someone mid-20s but as the rest of the face ages, it may not age as well regardless of weight or children. i always want to be cautious when working on someone younger because i want the whole face to work well together and be well balanced as it ages. i hope that makes sense. let me explain it another way, the fat that i put there does not go away. the rest of your face fat does. as the rest goes away from other areas of the face that were not transplanted, it may make my fat transplanted look slightly off. you will probably need more fat or fillers to balance my work in later years. that is theoretical.

Ok, I see where you're coming from. So theoretically one would need constant 'top-ups' throughout the following decades to keep things remaining balanced? If that happened, would the face not age better in general due to the lack of fat loss? And how often would one need these 'top-ups', do you think?

in general, it would probably age better but i rarely do people in their 20s. however, for those that i have done in their 30s and now i see them in their 40s without further intervention still look remarkably better so yes it can limit further aging.

i look at aging like a glass of water emptying. each year you get a little older. if you age rapidly you will need a more frequent top off. if you don't, you won't. that is the best way to understand it.

So, the main reason you are reluctant to perform fat grafting on those in their twenties is beacuse it would require further intervention? Is such further intervention to correct inbalances likely to be difficult and with no guarantee of success?

I guess what I'm asking, in short, is whether a face corrected of inbalances due to fat grafting in earlier years would look worse than a face that had aged without any intervention at all. If however it is simply a matter of the candidate having to undergo the inconvenience of several grafts in later life, then personally for me this would not be an issue.

no, further intervention is not the issue. in general, i just don't like doing a lot of permanent fills in someone young to make sure it ages well for them. the main issue with aging is weight issues in the future. i think if that is maintained that it should age fine. but obviously i don't know 100% because i do not do that many in 20 year olds.
