TCA/Silicone - Acne Flare-ups

This section of the forum is dedicated to discussions on managing, treating, and correcting acne scarring. (Of note, Dr. Lam is focused principally on acne scarring rather than on managing the ongoing condition of acne, which a dermatologist would be better equipped to address.)

TCA/Silicone - Acne Flare-ups

Dr. Lam,

Thank you so much for having this forum available. I am a 48 yr old woman and my acne is not completely controlled and I have been fighting my skin since I was 18. I am being treated, however my question is, can I start treatment anyway? My Derm is advising that first I have good control then laser tx, after reading all I have on your site and through these posts I get the feeling that going laser may not be a good solution, and if I am going to have the expense and do detriment I would much rather have the expense and do the best thing. While I know there is never 100%.. and maybe not even 50%, I am hopeful for some improvement. Based on the photographs attached could you give me some feedback please.
I have every kind of scar you have described, from icepick to rolling scars.. even on my nose, close to my lips and by my eyes.

hi,
personally, i would not do laser as it probably will not work. you are not a good candidate for silicone either (at least based on the photos). CROSS looks like it would work very well for you but it would require a few rounds to do it. i would probably spread the sessions out every 3 months and i am guessing (only a guess) that you would need about 3 to 4 rounds to make the difference. i think you could start treatment now even if you still are having some acne flare ups. if you don't go to me for CROSS please find someone with a lot of experience. it is something that you can easily mess up and make scars look worse.

Thank you, I wouldnt go anywhere else as I dont believe there is anyone with as much experience as you have.. and I cant afford to make it WORSE.

I do have one follow-up question however. You spoke a couple of times in 2007 about plasma tx. Given enough time for the "best outcome" of CROSS, perhaps a year (not sure when one sees the most results they could), would follow up with plasma afford any more benefit?

no, plasma will do nothing for acne scarring. i only really use it for skin rejuvenation. if i said in 2007 that it would help acne scarring, then that was old thinking and incorrect. :)
