What do you reccomend

This section of the forum is dedicated to discussions on managing, treating, and correcting acne scarring. (Of note, Dr. Lam is focused principally on acne scarring rather than on managing the ongoing condition of acne, which a dermatologist would be better equipped to address.)

Oh ok, good.
Yes, I know that the HA will disappear over time, they told me that it would last 3-6 months and its already faded quite a bit.

So you would just insert the silicon to where it looks good or would your over fill it knowing there is HA in there that will fade away?

I'm just know sure how many trips I can afford to make there because I'll have to fly in.........

i never overfill silicone. risk of it being lumpy because i don't know how much HA is there underneath. i would guess mentally budget 2 visits. hopefully you won't need a third or fourth. :)

Do you ever have silicon that just doesn't work on boxcar scarring for some reason?

yes, because scars are just hard to fix to one's satisfaction. i can improve but just not nail it. scars are hard to fix to the point that everyone is happy.

I understand, everyone wants to look normal and its not possible to have perfection but I guess what I'm asking is do you ever have no response at all from silicon injections into boxscars?
Not just not good enough for the person but do you ever have it not work at all? No improvement?

not that i can recall. it should provide at least some improvement to your condition. when i don't see a person live then i always withhold judgment since i am not reading your face in person but based on a photograph but i believe based on the photos you should be a good candidate.

Hey Dr Lam,
I just thought of something else I need to know..........I've had a bit of of success with needling........will I be able to do that after I have had silicon injections? Or would I risk puncturing the silicone?
I ask because even if the silicon injection raised the skin up to a level where it look good, the scar tissue still has that shiny look to it that which can be improved with dermastamping or needling.
So I want to know if once I do silicon will I still be able to do these other things?

good question. not really had that situation before. personally i probably would not do it after silicone. but if you did, wait at least a few months and always make sure your skin is very clean before doing it since you have a permanent filler there.

Oh, bummer. Yes I'm very careful to be clean and disinfect when doing it.
I guess the next question would be this: Do the silicon droplets puncture easily? And if so, and that happened during needling is it a health risk if the silicon gets in my system?
Or is that not any different of a risk then putting it in there in the first place?

no, it does not get punctured. it gets well integrated to the tissue. my only concern is that since it is a filler like any filler there is a minimal chance of infection. if you have needles that can be dirty on the skin constantly rubbing over the treated area my theoretical concern is possible infection. i know that may be very remote but i just want to express my concerns.

Ok gotcha, so if a needle gets put into the silicone, nothing happens......is that right? Is the silicone just really firm and/or solid then?
I made an appointment to come see you but I'm thinking I need to understand this fully first. Thanks

yes, nothing happens. silicone gets integrated into the tissues. you will not see any liquid seeping out if that is what you mean.
