depressed smooth scar

This section of the forum is dedicated to discussions addressing linear scars, depressed scars, keloid scars, hypertrophic scars, and other facial reconstructive topics. (Of note, Dr. Lam no longer accepts medical insurance so he rarely manages cancer reconstruction or trauma anymore.)

depressed smooth scar

Hi. i had a head wound many years ago, a big open mess of a scar on my forehead resulted . i've had 2 surgical revisions , the first created a linera scar with an obvious 'step' and some excess scar tissue remained. the second revision took it to a cleaner line but there was still uneven surface. over the years ive had laser treatments (10 in 2004) 4 dermaroller treatments in early 2010. By now the skin was much smoother and from some angles the scar didnt show but from others the line was still obvious and uneven. I then had 2 further laser treatments before xmas 2010. The second treatment went much deeper , and it actually left an indentation in the skin wider than the original scar and this indented area is smooth and slightly hyperpigmented. Ive attached a photo.
i wanted to ask what treatment route is best to take now. Would the stem cell 'spray' with ear punch graft work, as this is something recommended by a consultant near me. But i an wary that i'm being sold to as its a new treatment and vvery few places offer this in UK. Please advise.

since the scar line is not very detectable just color and depth are then i would not recommend surgical re-excision. i think the risk is too great especially since you have already had two prior cuts. i would say, injectables like restylane (temporary) or silicone (permanent, this should be done by a very skilled injector) could help raise the deficit then i would bleach the surrounding skin a little bit to match the color better. those are my initial thoughts.

thanks for the reply. to test the water with a non-permanent filler, would you recommend restylane over others like juvederm or radiesse, if so, why?

restylane or juvederm is fine. they are reversible and about the same. coke vs. pepsi in my opinion. i like restylane better but does not really matter. i would not do radiesse. it is not reversible and also i have seen unfavorable hardening of tissues under the skin over time. that is why i have stopped using the product.
