Skin reconstruction and Acne scarring

This section of the forum is dedicated to discussions addressing linear scars, depressed scars, keloid scars, hypertrophic scars, and other facial reconstructive topics. (Of note, Dr. Lam no longer accepts medical insurance so he rarely manages cancer reconstruction or trauma anymore.)

Skin reconstruction and Acne scarring

Dear Dr Lam,

Is Skin reconstruction an easier procedure compare to acne scar treatment ?

I have seen amazing before and after photos of Skin reconstruction on your website. I cant find scars in those post surgery photos.

My question is, why is acne scarring so hard to treat given that the skin injuries of Skin reconstruction patients are much more severe(deformities) ?

Is it not possible to achieve smooth looking skin through various treatments if i have acne scarring?

because acne scarring is all over the face, you cannot cut them out like a traditional scar revision. that is why it cannot be entirely corrected, only improved. but in many cases i can at least help it a lot and a lot more than other docs can in most cases.

Dear Dr Lam,

I understand that you would recommend TCA cross for ice picks and Micro-sillicone injections for bigger and deeper scars.

May i know where does the injected sillicone sit ? Right beneath the skin ?
How is it possible to accurately injected and place the sillicone at the right place without a cross section view of the skin ?

yes, you are correct. i have a ton of experience injecting silicone. there are two things that i go by: 1) i can feel the dermis give way as i enter the subcutaneous plane and 2) i know by needle depth which plane i am in.

Dear Dr Lam,

One of your patients had posted before and after pictures of his skin on a forum , after getting treatment with micro-sillicone and i must say the result is amazing.

May i know what is the rate of complications from injecting sillicone into the skin ?

As it is permanent, will infections or any side effects occur due to sillicone ?

What can be done if it happens ?

virtually zero if i do it. since i have tremendous experience injecting silicone i get very safe results. i also put only a little at a time and only use medical grade silicone. by doing those two things, i really create very safe results.

As it is permanent, will infections or any side effects occur due to sillicone ?
there is always a chance that a permanent filler can get infected. that is truly about the only risk that you have with silicone.
What can be done if it happens ?
if that happens, you would need long-term antibiotics like 4 to 6 weeks and 5-FU injections that help also. this is very very very rare. i have personally never seen it, and i have done thousands of patients. also, i know those who have done it for 40 plus years have not encountered even a single case. however, it is possible and you need to know of that risk just like any permanent filler.
