Facial fat grafting and acne scar

Greetings Dr.Lam,

I am 29 with a gaunt, very tired looking face... i developed that over the last 7 years and nothing seems to fix it. It actually started when i lost about 16 pounds and my face went from nice looking and fresh to this sick look...i went from 116 to 100 pounds and i am 5 7' ( i barely gain any weight ) so i wanted to see if i can have a fat transfer ...how much would it cost? and is it a good option for me? would it return my look back to me or at least near what i looked like since people can barely recognize me?

Also what would be a good treatment for my acne scars.....its not so bad but i need to fix all that ...its really making me sad all the time.

hi dina,
1. fat grafting is not great for tiny acne scars because the product is too soft to lift up acne scars plus it is a metabolically active product, which can change with significant weight gain or loss and with aging, which i will discuss in point #2 below. for acne scars, i prefer silicone to manage them if they are bigger holes and CROSS acid treatments for tinier holes.
2. in general, i am not a big fan of doing fat transfer in someone younger than 35 because i am always wanting a fat transfer to age well for someone. the greatest fear i have is weight gain as your metabolism slows down. fat transfer looks great and is permanent but can look bad if you gain 15 to 35 pounds after it. to be absolutely safe, you might want to consider restylane or other products that are temporary but long lasting in your age group. i can do a conservative fat transfer if you understand that you must maintain your weight over your lifetime. knowing if your parents have remained thin during their lifetime is a helpful indicator. if you have all of a sudden lost weight that is a bad sign because you may gain it back then the fat may not look as good. hopefully all of that makes sense.

Hi Dr. Lam ........thank you so much for your prompt reply , makes me feel much better about my situation. In connection to point number one my acne scars are not very bad they are more like darker spots that adds to the unhealthy look i have now.

I have already gained about 70% of the weight i lost and i never go above 58 kilos anyways , i am always under weight and yes its a very common thing in my family that we are underweight......but my face is still 4 times less in size than it used to be, I would like to see if there is a way to send u a picture of me now and be4 cus i feel ill never get back to what i was. Its really scary that my features have really changed even my skin texture and complexion.

Is it ok to ask for an approximate cost ? as i do wish to get done with that soon
