Hi Dr Lam:
I have an interest in getting revision rhinoplasty and have heard great things about your practice. I am a black female who had an open rhinoplasty in Oct. 2005 and have dislike my "bulbous/thick" tip ever since. I have read online and believe I have a soft tissue pollybeak deformity. My tip feel as if it's still swollen and a little bit hard. I also don't like my nostils and believe I have deep alar grooves<==you can tell I've been on the internet. I would like to have the tip raised and the nostrils brought in/reduced. I have attached a pic link because I wanted to make sure this type of revision was something you may be able to correct. Of course, I know a face-to-face consult will be needed so I'm planning to come to the Dallas area in August. **Note: I'm also interested in reducing my non-feminine, unshapely lips but I guess i'll post that under the appropriate forum:p...Thanks so much

hi,
thanks for the photos. here are my thoughts:
1. as far as the bulbousness of the tip, you probably understand that further reduction may cause more bulbosity. sometimes we can only get the tip a certain size. think of the idea of the tent analogy. hope you saw that video regarding ethnic noses. if not here it is:

with that in mind, i had a hispanic woman who had an overly reduced nose that had a soft-tissue pollybeak. i corrected it with perfectly injected 5-FU injections with 6 weeks of nightly taping. i did that twice and had a huge correction. she was from another city in texas and travelled twice to get it done. that does not work on everyone but it might in you.
2. as far as the deep alar groove, that is really tough to fix. there is not really a lot to do for it. you can put a graft to stent it but that would cause the nose to be wider and bigger. it is very hard to inject that area. if you don't know why, watch this video:
