Under-eye hollows - Results with fat transfer vs. restalyne

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. Under-eye hollows - Results with fat transfer vs. restalyne

Howdy Dr Lam,

I'm a pretty average (maybe on the thinner side) 36 m, and I've seemed to have developed hollowing of the eyes over the last few years, as it runs in my family. I've exhaustively researched the different methods, techniques and procedures (probably more than I should have) and I've known of your success with your patients for quiet some time. If I get anything done involving my eyes, I know for sure I will choose you. So with that, my questions are as follows:

1.) While I know that your fat grafting is probably what I want and need, I'm sure it will be pretty expensive compared to using a temporary "fix" such as restylane under the eyes. I've read several times that this is the product of choice for under-eye treatments. However, can good results be achieved with restylane if you perform this? If so, albeit only temporary, are the temporary results similar or comparable to what can accomplished with a fat transfer when it comes to getting rid the hollowing appearance?

2.) I seem to have a few tiny veins that love to show themselves within my eye hollows which makes their appearance even worse. If I'm kinda "veiny" should I expect more bruising after injection or a fat transfer? Would having these tiny veins under my eyes complicate or inhibit any of the procedures?

3.) Would it be possible to do a fat transfer under the eyes with only local anestesia?

thank you for the kind words. here are my thoughts in caps:

YES, THAT IS CORRECT. I've read several times that this is the product of choice for under-eye treatments. However, can good results be achieved with restylane if you perform this? YES, IT DEPENDS ON THE HOLLOWNESS. VERY HOLLOW EYES DO BETTER WITH FAT. GIVEN YOUR AGE AND SINCE YOU ARE A MAN, RESTYLANE MAY BE A GOOD INEXPENSIVE BET. WITH WOMEN, THEY TRULY WANT TO LOOK YOUNGER SO RESTYLANE IS NOT AS GREAT AN OPTION BECAUSE IT CAN FILL A HOLE OR TWO BUT NOT REJUVENATE LIKE A FAT TRANSFER. If so, albeit only temporary, are the temporary results similar or comparable to what can accomplished with a fat transfer when it comes to getting rid the hollowing appearance? SEE ABOVE.

NOT REALLY BECAUSE IF YOU ARE HOLLOW ENOUGH, I USE CANNULAS FOR BOTH RESTYLANE AND FAT TRANSFER SO THERE REALLY IS NOT MUCH DIFFERENCE. SOMETIMES FOR MINOR TEAR TROUGH HOLLOW OR TOUCH UPS I USE A SMALL NEEDLE THEN THEORETICALLY YOU COULD HAVE MORE BRUISING BUT I PRE-ICE AND CAREFULLY AVOID VESSELS BUT STILL COULD NOT GUARANTEE NO BRUISING.
