Hi Dr. Lam. I am coming to see you in a few weeks for fillers and botox. You graciously helped me via a phone consultation many months ago. I have some questions before I arrive, though.

You agreed that I am in need of a fat transfer but I had radiasse injected under the eyes in January and your staff said that I have to wait a year before you can do the fat. Would you be able to do a partial fat transfer and then inject the other areas after the radiassee disappears?

Also, Can fillers be injected in the lower cheek region and chin? I see that I have depleted fat everywhere ....under my eyes...around my eye sockets, temples, lower face (jaw line is narrowing significantly)... I am only in my thirties...but I am really suffering from fat loss. I'm really desperate to get this fixed asap...

My big concern is that the fillers will look fake when I smile. I have seen your fat transfer patients and they look so natural...but will fillers have the same effect? Everytime I've had fillers they alter my smile and make me look fake and chipmunk like... I work in a very visible profession and I need to fix this soon -- and have it done well.

the radiesse definitely puts a damper on things. it makes things hard to read because sometimes radiesse lingers for years. the more radiesse you put in the more scar tissue develops almost like a cement so please refrain from any further, especially if you are considering fat transfer.

secondly, the more fillers i put in there (of any kind) makes fat transfer even more difficult to figure out as we proceed. i really encourage those who are interested in a fat transfer to stop all fillers if they have not had too many and move to fat when the time is right.

given what you are discussing i would highly recommend waiting out at least 6 months from the radiesse (send me a photo unless you are coming down here) and then just move to fat with the limitation that i would have to put fat somewhere where the radiesse is. sometimes we get lucky and we don't need to go back there if the radiesse has absorbed already or is stable and does not go away. again i discourage more and more fillers if you need fat transfer.

first, when i do cheeks alone with fillers, i use very little to avoid that chipmunk issue. when i do fat, i can do more to make it look younger but then i have the luxury of blending that cheek into the surrounding areas like the buccal, subzygomatic, temple, tear trough, etc. so that the cheek does not just stand out like a sore thumb.

if anything if you come down here, i may just do botox and do a proper evaluation and try to move you to fat transfer in the coming months. we can discuss.

Thank you so much for your feedback. So you think 6 months from the time the radiasse is injected? Oh, that would be wonderful! I had it the first of January...so do you think I could have the fat transfer in June or July? I would be thrilled. A whole year just seems like an eternity when you are READY to do this procedure. Especially, considering I am in the public a lot because of my career.

I know you mentioned that models often come to you even at a young age to correct facial hollowing. Can you place the fat in the eyebrow area too? I just have a skeletal/haggard look when I get at the weight I want for my BODY. It's so discouraging. 5 or 10lbs makes a huge difference.

Also, another surgeon suggested that I receive a chin implant. I don't have the time to be out of the public eye for that long, but he mentioned that fillers can fix the minor problem that I have with a recessive chin. Could you do THAT procedure when i'm there in April or should I also wait for fat transfer in that area, too.
