Scar Revision

This section of the forum is dedicated to discussions addressing linear scars, depressed scars, keloid scars, hypertrophic scars, and other facial reconstructive topics. (Of note, Dr. Lam no longer accepts medical insurance so he rarely manages cancer reconstruction or trauma anymore.)

Scar Revision

Hi Dr Lam,

I have a indented scar on my forehead, right above eyes. I had surgery many years ago, over 15yrs ago, one surgeon butchered me, than I had another save me. But after taking on such a horrific scar, and after a few months, the scar opened became indented. I have rough edges which make it hard to cover with makeup, looks more noticeable. anyway now for the present, two years ago I decided to give it one more try, to my many unsuccessful surgeries. I went to see another surgeon, very hesitant I was, I explained my situation that my skin does not react well to dissolvable stitches, he convinced me that 10 years ago it was different, honestly I thought he would laser it something else. Well after the first scar revision, not only did he make longer, cause he said he had to, but wider and so horrible, he decided he wanted to wait 4 months and perform another scar revision, again now worse... he then did a quick laser.. OMG!! anyway I decided a month ago, he was revered to me by a client. He is head surgeon of two very good NYC Hospitals, I went to see him for a consultation, explained all the unsuccessful surgeries. He was the only Dr that said before anything, he would freeze my entire area with botox. But he only wants to do half of the scar, just to see. Dr Lam I just want to know, should I just learn to live with this scar and be happy. Is it that my skin maybe cant take yet another surgery. Is it too soon, since the other surgery was 8-9 months ago.. Dr lam please tell me this sounds right, should I try this procedure, with botox give it a try..Or what would you do different. very very nervous and have lost TRUST in surgeons..sorry.

hi,
i am sorry to hear of your situation. i should not give you direct medical advice on this forum but i can speak in generalities:
1. sometimes scars don't heal well because of the tissues. sometimes the results are not as perfect because of the previous surgery. that is hard for me to say without really having taken care of you even if i review your situation more indepth.
2. botox in my opinion is an excellent idea since i use it in almost every one of my scars. it helps because it does not stretch the scar out further and also it helps because somehow the scar softens just from the presence of botox.
3. whether the 8 month time interval is sufficient is hard to say. it certainly is somewhere in the gray zone.
4. whether to do half the scar or not is a judgment that only your surgeon can tell you but it is not unreasonable to be more conservative.
5. whether you should do it or not is depending on your emotional state and how visible the scar is to a bystander, not necessarily to you. all of this is a highly complex subject. one thing you can do to help yourself is to give yourself forgiveness for doing the first procedure and to get your emotional state clean so that you can make better judgements. watch these videos as a start:
