This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

Hi Dr. Lam,

I would be extremely appreciative if you could take a look at my photos and tell me your opinion on whether or not i would benefit from fat grafting in the tear trough area, or if not fat grafting, if any other procedures would be of benefit. I'm supposed to have fat grafting done (in South Africa) in February, but of course I still have some doubts and Scepticism. From watching your youtube videos and browsing your website I have come to highyl respect your opinions on these matters and would really appreciate any advice you could share with me.

Thanks very much and happy new year!

of course, reviewing photographs only without an in-person evaluation is very far from being accurate. in general, i do see some hollowness going on. perhaps you also have a little swelling under the eyes as evidenced by the light pink color. that could be a manifestation of allergies or some other condition. whenever i see a younger person come in with some abnormal coloring, possibly swelling under the eyes, i always look for allergy symptoms, thyroid conditions, etc. before considering them for a surgical option. i don't know your clinical history and am not about to explore those options with you. i think it would be more appropriate for you to check with a physician locally regarding those matters.

given the fact that you are just a little hollow, if you go with fat grafting be very conservative. there is a fine line between a good result and a problem. if i were treating you, i would go very conservative on the fat. i would also explain to you that given what i see, you would STILL have some remaining hollowness given my my conservative approach in looking at your anatomy. however, i would consider exploring the medical issues first. i would also not graft any other part of your face. it looks fine as far as what i can tell in the photos.
best,
sml
