Alarplasty scar reduction

Alarplasty scar reduction

Hi Dr. Lam.
I am Asian and had a rhinoplasty including an alarplasty done (overseas) about 7 weeks ago. While I am happy with the overall result, I have some visible redness and scarring from the alarplasty on my left side. The area in the left alar groove starting at the base of the nostril has a small raised scar about 5mm in length. Something more problematic is an indentation about 2mm in diameter at the end of that line scar -- where 2 extra stitches were put in by a junior doctor 5 days post-op because the original set of stiches were not healing properly. Those stiches pulled the surrounding skin downward and so they healed into an indentation.
My questions are:
(1) Are these complications common after alarplasty?
(2) How much will go away naturally over time?
(3) How soon after the surgery should I consider options to reduce the scar and indentation -- does it make sense to visit your office now or wait a few more months?
(4) How would you treat these issues?

My questions are:
(1) Are these complications common after alarplasty? UNFORTUNATELY YES. THAT IS WHY I GO THROUGH THE RIOT ACT WITH MOST PATIENTS BEFORE ELECTING AN ALARPLASTY. I ALSO DEFINE FOR THEM WHAT TYPES OF ALARPLASTY I WOULD DO BASED ON THEIR ANATOMY AND WILLINGNESS TO TOLERATE RECOVERY AND RESULTS.
(2) How much will go away naturally over time? THAT IS SOMETHING YOU NEED TO ASK YOUR SURGEON. YOU KNOW THAT I MAKE IT A POINT NOT TO ANSWER PEOPLE'S QUESTIONS ABOUT RECOVERY. IT IS NOT FAIR TO YOUR SURGEON OR TO YOU.
(3) How soon after the surgery should I consider options to reduce the scar and indentation -- does it make sense to visit your office now or wait a few more months? 1 YEAR MINIMUM TO SEE ME BUT SEE BELOW...
(4) How would you treat these issues?
AN ALAR SCAR IS VERY HARD TO MANAGE. DERMABRASION CAN HELP BUT IT IS NOT A PERFECT FIX. YOU NEED TO TALK WITH YOUR SURGEON TO HELP GUIDE YOU FOR THE NEXT FEW MONTHS. LET HIM/HER WORK WITH YOU TO IMPROVE YOUR SITUATION.
