Finally Back!!!!!! and non surgical nose jobs

This section of the forum is dedicated to discussions on rhinoplasty (cosmetic, reconstructive, and functional) nose surgery, also known as a “nose job”. Ethnic rhinoplasty topics can be addressed either here or in the Ethnic Cosmetic Surgery Forum.

Finally Back!!!!!! and non surgical nose jobs

Postby Mysteryagain » Thu Aug 27, 2009 8:55 pm

Oh Dr Lam, I missed you badly!!!!!!!!!!!!!!!!!!!!!!!!! and Memento, and everyone here!

This year has been awfully busy and when later on I tried to get online (log in, actually), I could not, who knows why! but I finally got around it and I am here!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

How is everyone doing?

Well, this is not the forum for the chatting, so I will have to post elsewhere and... get updated with all the entry blogs :)

But I will start by making a question (as usual  )... when people have non surgical nose jobs with fillers for correction of the tip of the nose, does the filler have to be necessarily injected *in* the nose (up the inside of the nostrils and up/in the collumela, etc.) AND the nasolabial angle, in ALL of those areas? Or... can the filler be used only for the nasolabial angle and the skin righ below the nose, the nose-to-lip skin, without injecting inside the nose in any way?

I am asking because I fear that injections of fillers to lift the tip of the nose that inject some substance up the nose, even if from the inside, could make the nose bigger or more bulbous, or the nostrils different, while in some cases, it might be needed, only (I say MIGHT, I have no clue, just asking) to make the skin that runs from the base of the nose to the lips represent a strronger "base· to sustain the tip of the nose...?
Say, in my case, I believe that any filler inside my nose could make my nose tip bigger looking or my nose longer looking, while the effect I want is simply the little uplift that I get when I pinch underneath my nose and pretend have a bit more skin (and not little skin and a slope) at the base of my nose. When I do that, my nose does not get bigger but the tip gets lifted a bit, just right... I wonder if it is possible then to look for having a filler injected there and only there?

What do you think?
Or does any filling of the type require some filling "inside" the nose?
I am sending you pics by PM :)

welcome back!!! as far as fillers at the nasolabial angle are concerned, yes they work for people with a hanging nasolabial angle. that is known as a premaxillary augmentation. it does not rotate the nose but creates the illusion of rotation. i tend to like that technique (but i typically use crushed cartilage from the septum of the nose) to create my premaxillary implant in conjunction with a rhinoplasty. if you google premaxillary augmentation, my article on the subject should come up #1 or 2. the premaxilla is a narrow bone that sits between the nose and the lip. in the past, i used wide silicone implants but had problems with them since they affect one's smile so i had to remove most of them. most people with a premaxillary deficiency are ethnic, particularly Asian and African. as i reviewed your profile, you actually have a good nasolabial angle but a more protuberant premaxilla. i would say if you put product in there, you would actually have the premaxilla way too far forward. look at your profile pic again.

just as clarification, injection of product into the tip of the nose itself typically yields very very little results compared with the top bridge of the nose. the skin of the nasal tip is firm and adherent. you can't get product under there so it tends not to move and create a good result. so the area you are asking for is much better for injectables but not in your case.
best,
sml
