acne scars-silicone

This section of the forum is dedicated to discussions on managing, treating, and correcting acne scarring. (Of note, Dr. Lam is focused principally on acne scarring rather than on managing the ongoing condition of acne, which a dermatologist would be better equipped to address.)

dear dr lam,
i have recently had 2 rounds of silicone injections by dermatologist in NY. i can see improvement but i also had some lumps which looked like pointed fybrotic little grains in the epidermis. i had most of them removed quite well with electrodessication surgery, i had small lumpy area left on the right cheek therefore visited different derm on monday who treated these 3 or 4 small lumps also with electrodessication. i dont know what has gone wrong but he didnt seem to target the right spots and i have now developed cyst or abscess which i am beeing treated for with antibiotics and might end up having it drained surgically. it seem to be a no win game for me at the moment. i believe silicone was injected to superficially in first instance. i am sure this little infection problem will get sorted. unfortunatelly the lumps will most likely stay in. would you be able to give me some advice on what can be done with these little lumps? its very small area on the right cheek (atrophic) and it was previously very well filled with restylane. now its atrophic and lumpy. i dont know what to do , laser, dermabrasion or continue with electrosurgery or just more restylane to camuflage the lumps? thank you for your kind advice

hi,
that is not easy to fix other than cutting it out. fortunately, i have only seen this once a few years ago when i injected a person in the nasolabial groove. that is why i don't inject it there anymore. i just shaved it down. i get very accurate injections just past the epidermis so i rarely if ever see such issues any more. i wish you the best in management of it. sorry to hear what happened.
