Skin quality anf fat loss under eyes

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

Skin quality anf fat loss under eyes

Hi Dr,

I have two questions:

a) is it possible that sometimes "bags" under the eyes can actually be a form of hollowing, or something that can be corrected with fat transfer. all the doctors I see say I have hollow eyes and i feel i can see this when i look straight into the mirror, but when i lift my face up and roll my eyes down to look at my face then I appear to have sort of "bags" under my eyes

b) Can losing fat under the eyes diminish the quality of the skin. Is the skin more prone to wrinkling, and is it possible that more veins will begin to show through, because there is no fat propping the skin up. And can this be corrected with fat transfer?

my responses are below IN ALL CAPS:

YES, YES, YES, YES, YES. PLEASE SEE MY VLOG IN MY MEDIA CENTER IN WHICH I TALK ABOUT THE LOWER EYELID LIKE A SHORELINE THAT RECEDES SO AS TO EXPOSE THE ROCKS (THE EYEBAGS) SO TAKING OUT THE ROCKS IN MY OPINION IS UNNECESSARY IN 9 OUT OF 10 PATIENTS. SIMPLY FAT GRAFTING THE LOWER EYELID IS SUFFICIENT. EVEN IN THAT 1 OUT OF 10 CASE THAT NEEDS EYEBAGS REMOVED, THEY ARE STILL HOLLOW AND REQUIRE FAT GRAFTING. I AM GLAD THAT YOU ARE SMARTER THAN 99% OF PLASTIC SURGEONS OUT THERE WHO STILL DO NOT "GET IT". OKAY, SORRY GOT A LITTLE EXCITED THERE.

I THINK ANYTIME WE TALK ABOUT FAT GRAFTING FOR SKIN QUALITY, WE START TO ENTER A WORLD OF CONJECTURE. AND I DON'T LIKE TO OFFER YOU GUESSWORK. THAT WOULD BE MEANINGLESS. I THINK TWO THINGS MAY HELP TO INDIRECTLY ANSWER YOUR QUESTION:
1. THE HOLLOWNESS APPEARS AS A SHADOW AND THE UNDERLYING CHEEK HOLLOWNESS EXACERBATES THAT LOOK SO THE SKIN WITHOUT LIGHT AND WITH MULTIPLE POINTS OF SHADOWS MAKES THE LOWER EYELID SKIN LOOK WORSE.
2. I HAVE SEEN IMPROVEMENTS IN SKIN TEXURE AND TONE WITH FAT GRAFTING (AS MENTIONED ELSEWHERE IN THIS FORUM) BUT AGAIN WOULD EMPHASIZE THAT THIS WHOLE STEM CELL IDEA FOR SKIN CHANGES HAS YET TO BE DEFINITIVELY PROVEN IN MY OPINION. ULTIMATELY, SUN EXPOSURE, BOTOX, SKIN THERAPIES, AND SIMPLY GOOD GENES DICTATE MORE OF HOW THE LOWER EYELID WILL APPEAR AS YOU AGE.
