Scar Revision/Realignment

This section of the forum is dedicated to discussions on managing, treating, and correcting acne scarring. (Of note, Dr. Lam is focused principally on acne scarring rather than on managing the ongoing condition of acne, which a dermatologist would be better equipped to address.)

Scar Revision/Realignment

Around a year and a half ago I was involved in a car accident and I was left with an inch long scar. Unfortunately the scar left was more or less across the center of my forehead, almost imagine half an X something like \ if that makes sense. It was also stitched up in a relatively poor fashion and as such healed in a way which left it widening a bit. The scar has faded a lot over the time, but still the location of it obviously bothers me. I am ok with having a scar, but am really hoping it can be improved upon.

Ideally, and I don't know if this is possible, I would like to have the scar realigned away from the center of my forehead, and also closed up in a cleaner manner. I don't know if it is possible, without lengthening the scar greatly, but I'd be willing to take the time to gradually relocate the scar, assuming the skin would eventually stretch across as it would happen, and finally have it closed up in a cleaner fashion. Again, I don't know if it is possible, but I am looking for some insight and hoping. I have posted some pictures below. The first one is relatively soon after the incident (the scar has faded somewhat since then) and the second one is potentially I way I'd like to have it relocated. realigning a scar involves using a z-plasty technique. unfortunately, a z-plasty also lengthens a scar, which i agree with you would be unnecessary and unfavorable in your situation. i use z-plasties primarily to lengthen contracted scars and realign very poorly oriented scars, neither of which is the situation in your case. the ferrari of a scar revision is a geometric broken line repair. here is a link for more info:

to get a great scar revision, you would require these things:
1. pre-botox done a day or a week before the procedure (botox limits movement during healing and muscle stretchback).
2. a great GBLR repair
3. post-scar revision management (possible laser and/or 5-FU hopefully not at all or once or twice if the scar thickens)
4. dermabrasion at 6 weeks to 3 months to smooth any residual tiny edges out and to blur the scar further.
5. patience because it will look worse again before better. you would have to give it another year or more of looking worse before looking great.
