can perlane/fillers lift lower cheeks?

Postby rvaw » Mon Apr 20, 2009 1:53 am

Hi Dr Lam

i'm a 30 yr old man. i have full chubby lower cheeks making me look a little grumpy but my upper cheeks lack some volume.at first i thought about getting liposuction to the area with buccal fat removal but now i'm considering having some fillers in the higher cheekbone area but want to know whether this alone will help my skin tighten and lift the lower cheeks to give a more pleasing contour rather than just make the upper cheeks chubby as well.

i'm sure actual cheek implants would solve my problem as they're solid and would definately lift the area and looking at my face i'd probably need the smallest ones, but i'm not so sure about it as i'm concerned that it may look like i've obviously had it done which isn't great if you're a guy.

i'm also wondering whether you can have any other procedures done soon after perlane/restylane/sculptra or do you have to wait a certain amount of time. i'm also fairly dark skinned so does that affect what treatments would be suitable for me?

sorry for all the questions but i'm fairly new to this. i'd greatly appreciate any advice.

thanks.

first, for the fill versus lift business watch this video: http://www.lamfacialplastics.com/plasti ... eo_id,539/

second, i don't believe in cheek implants for the majority of people because as you get older the implant becomes exposed. you might be an exception. i would have to see you.

i don't know what other procedure you are talking about to wait for after your injectable? you need to clarify.
best,

thanks for your reply dr, the video cleared things up for me. i have not had any fillers or treatments done yet as i'm still in the 'research' stage and decided to contact you as i've not come across any surgeon or website with as much detail and breakdown of facial aesthetics. i'd love to have a consultation with you in person but i live in the u.k., looking at previous posts many of you patients do travel to see you and that's something i'd like to do.

if i may just let you know of my current situation, i've recently lost weight and am now the correct weight for my height but have retained chubbiness in my lower anterior cheek area going down to the sides of my mouth. this area looks very full when i'm not smiling giving me a sad/upset look. as i lost weight i noticed that my cheekbone area(on the sides in front of ears) was slimming but this lower anterior cheek was not. this has caused a 'transition zone' between the 2 areas and just looks like a small dent on either side. my face also has quite a bottom heavy look because of this. as i've seen in your video the 2 finger rule does seem to improve things but i'm not keen on a facelift at this age.

i do still look pretty young for my age though but i'd rather look a little more manly and masculine now. the cheek implant was of interest as i thought the structure would help hold the skin up and provide some support. i am interested in a consultation even it's an online one to start with and i'd greatly appreciate any advice on the best way to go on this.

thanks

i can see what you are talking a out.
a few thoughts:
1. a cheek implant would help you build a little upper outer cheek but you would still be hollow in the central upper cheek and lower eyelid area. only fat transfer can safely restore that area.
2. having lost weight and some of your hollowness is due to that, i don't think you are safe to do a fat transfer given your age and your weight changes.
3. your lower outer face is a bit full. i always check to make sure that you don't have something easy to fix like a hypertrophic masseter due to the way you were born or due to teeth grinding. you can bite down a feel your muscles on the outside. if that is the case, then botox can reshape your lower face.
in conclusion, either a small cheek implant or a start with restylane/perlane to cover both the outer and central upper cheek might help.
best,
