fat grafting in upper eyelid after asian bleph?

Postby miaou5 » Thu Apr 02, 2009 7:46 am

after my asian bleph, i noticed that my eyes had become too large...buggy, you might almost say. other than a revision bleph, are there any alternatives to lowering the upper lid by a little bit? would fat grafting/filler in this area work?

that said, most of the time i see revision blepharoplasty being used to correct the crease size, but not the actual size of the eye. could a revision bleph help make my eyes smaller? thanks!

yes, that is possible. i really correct believe it or not most of the times over cut and overlifted eyelids/brows for caucasians more frequently because more often than not they also look bad with overcut eyes. i have attached an example of a lady who is 9 months out from fat grafting to the face and silicone to the lips to correct 4 problems: overlifted brows, overcut upper eyelids, overcut lower eyelids, cheek implants sticking out, and a bad facelift. if you are under 30 years of age with an asian eyelid problem, my fear is weight gain later in life because fat grafting is a live graft. however, the good news is you would continue to lose fat in the brow anyway so you should be fine in my estimation. i really don't believe anything else works well other than fat transfer there. if you are older, then i get to rejuvenate the face as well. if you are not, then i won't.
best,

not exactly because once the crease is formed, you will have a different eye shape. i would just aim to make the crease height smaller and the eyelid shape fuller.

can you do a bleph and graft simultaneously? my single lids are naturally becoming hollow due to genes.

typically i prefer not to do a double eyelid the same time as a fat transfer because i put fat in the upper eyelid and do not want to have the fat put strain on my fixation sutures. i would do the double eyelid first then wait 3 months to come back to do the fat transfer if you want both.

what about fat relocation rather than fat grafting (combined with asian bleph)
is recovery time for fat grafting the same as for a bleph procedure?

I personally do not believe that method works as well. Obviously I could put the fat at the same time but I am worried about the strain of the fat pushing on the newly formed crease. I am just being more conservative.

well, i am not really sure I need a fat graft, but my eyes are naturally "buggy" so that my eyelids fall into a natural indent about half an inch above my eyelash line. Obviously I think that's too high for a crease placement, and ideally, I'd like a crease halfway between the two (about 1/4 of an inch above my lashline). However, if you placed a crease there, would the indent just above it cause problems holding the crease? (such as creating multiple folds, etc...)? would you need to insert fat into the indent?

i would have to see your case. but in general, that indent should not be an issue. it is a reflection typically of a weak partial fold.

it's not a partial crease but an indention where my brow bone meets my eye socket. i've attached a picture to illustrate:

ah, got it. that is due to 1. aging, 2. thinness, or 3., just the way you were born. fat transfer would be one way to address it versus like restylane which would be temporary. i believe that adding volume there without balancing the lower eyelid and mid face and chin can make the face look off in many cases. at some point, i should review your case.

I know you do not like to do the suture method, but do you make any exceptions? I believe I am a good candidate.

hi,
i am truly sorry but i really don't. the failure is too high and i stopped carrying the sutures to do it. i am sure you will find someone to do it for you. :)
