Three questions about facial fat grafting

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

Three questions about facial fat grafting

Hi Dr. Lam,

I think you have beautiful results and I am strongly considering having fat grafting to my face. I have three questions and I was hoping you could take a look at them... thanks!

1) I have heard that facial fat grafts last longer if they are injected into the muscle or in "layers" (I am not sure what that means). Is this something that you believe is true?

2) Do you find that you have better luck with fat harvested from some areas vs. others? I read that some doctors like to use fat from inner thighs or lower abdomen because it lasts longer, as the fat composition in these different areas is different. I am wondering what your opinion is about this.

3) I have pale, sensitive skin... I am wondering if patients ever have scarring at the injection sites.

I think facial fat grafting could really help me. I had an illness a few years ago in which I got very skinny and lost a lot of facial fat. The fat came back in the rest of my body but my face still looks sort of gaunt in my cheeks, undereyes, and temples. I am 28 years old.

Hi,
1) I disagree completely about the muscle issue for two reasons. First, the guy that invented that technique performed that technique in the office of my fat grafting co-author's place and the fat went almost completely away. My colleague redid it in the subcutaneous plane and the fat now has held beautifully. Therefore, placing fat in muscle in my opinion really does not add longevity. Besides, I get tremendous longevity so I am not interested in changing the formula.
2) In short, yes. Those areas are the best, and that is where I go (lower abdomen and inner thigh) in the majority of my patients.
3) I really have not seen scarring in the injection sites. Remember all I am doing is using a standard needle to make the 6 entry sites.
