How Do You Know How Much Retains?

This section of the forum is dedicated to discussions on various surgical techniques for facial rejuvenation, including fat transfer/fat grafting, browlift, blepharoplasty (cosmetic eyelid surgery), facelift, etc. Also, this category includes questions on hand rejuvenation via fat grafting. (Of note, Dr. Lam does not perform body rejuvenation except for hand fat grafting)

How Do You Know How Much Retains?

Like I've said in prevous posts I have a gaunt face but when I smile this area fills in nicely. So my question is if I get fat transfer, will my cheeks be too big when I smile and cover up my eyes practically from too much fullness?

Also have you ever had a patient retain say 90% of fat and the it's too much? How can you tell beforehand how much someone will retain? I am please with my appearance overall and am an attractive person. I know some cheek fullness will make me more attractive but I am very concerned about turning out less attractive my having too much fat or it retaining more than you anticipate. Have you ever had this problem? Do you find it common for patients to be very concerned about the result?

1. i really can't tell you how much a face retains in terms of percentages. however, what i see is very good retention in terms of how much i put in. in other words, i can predict a result based on what i put in. i know if i put this much fat in, i will get this kind of result. however, i don't know if it is a 1:1 or less. i just have tremendous experience knowing that if i put in a certain amount in a certain way i just get blow away great results.
2. interestingly, when we get older the lower cheek is what gets too full with weight and aging. i blend the upper cheek to create more uniformity. if you are younger and like the way you look when you smile (which i have to see to determine that with you), then i can either adjust down the amount or not even do it. i would again have to see you in person to help you make that decision.
3. i have found pretty consistently great results. i really have not seen overstuffing a face. if anything, i have seen a need to put a little more in but that is quite rare. i hope that is clear. bottom line is you need to trust me to do the right thing for you when you are ready. these concepts can be refined with a consultation.
