two things: 1. shaving it off will almost definitely fail because you will miss half the mole that is deep. this is not a flat mole so it will come back. 2. if you cut it out, yes you are taking a risk with distortion to your lip.
my feeling is that these moles are unsightly and would merit surgical excision despite the risk of lip distortion. i would have to mark you out and look carefully but i feel that i could do this without huge risk but yes there can still be some distortion to the lip. another option is to shave it and hope the area contracts down a little bit then do an excision several months later to see if you can get a smaller excision done. there are honestly probably no other significant risks other than you will have a "scar" there, meaning a line that hopefully over time will be hard to see if done right.
