Jawline Scar

This section of the forum is dedicated to discussions addressing linear scars, depressed scars, keloid scars, hypertrophic scars, and other facial reconstructive topics. (Of note, Dr. Lam no longer accepts medical insurance so he rarely manages cancer reconstruction or trauma anymore.)

Jawline Scar

Hi Dr. Lam,

I have an 8 year old scar on my right side jaw line that was diagnosed as a keloid by two different dermatologists(although it never grew larger than it original was throughout the years). The scar was a result of numerous acne and abscess/boil development in that area. I've had a lot of Kenalog shots administered in different parts of the scar over the years that has left a small "cluster of scars" and I also noticed slight atrophy. I've also done v-beam laser treatment for the redness and it seems to have worked for the first few months and then the redness came back completely. I am Chinese, by the way.
Can you please tell me if my scar is truely a keloid and if there is any treatment that can improve the scar's appearance, color wise? Would excision be a possible treatment to reduce the scar?

unfortunately, there is no easy solution to this. i think it will take multiple methods to get it better including
1. silicone micro treatments to the holes and dips
2. bleaching product to manage the hyperpigmented color areas
3. micropigmentatation (that is tattooing) for the hypopigmented white areas.

despite all of this there will only be improvement but not elimination of the problem.

Thanks for the reply.
Do you also think the scar is a keloid?
When you say bleaching do you mean hydroquinone?

I understand elimination is not possible but I want to minimize the appearance especially in the sun. The redness is my biggest concern.

1. no, this is not a keloid. it is a hypertrophic scar.
2. i would also use regenica which is a human fibroblast serum. it really helps to manage old scars. i can send you one if you can't get it.
3. bleaching is a combination of either my AM Product, hydroquinone, tretinoin, etc. multiple products that have different pathways work better.
4. redness is going to be the Regenica product, my AM product (ova-skin.com), Vbeam or KTP and not picking or playing with it.

Thanks a lot for the recommendations. I will look into your AM products. Vbeam results were not very good for me but I will look into KTP.

What do you mean by "manage old scars" with Regenica?

most old scarring cannot be managed with creams but regenica can soften (not eliminate) your older scars.

Dr Lam,

Sorry for restarting this thread. I was told fraxel laser can help smooth out the scar, helping with the texture. Is that something that you think might work?

fraxel will do little for atrophic scars and will worsen hypertrophic ones. in general, not a good idea for either situation.
