I have reviewed your photos and you are an excellent candidate for fat transfer. As you can see, you do no need any eyelid surgery whatsoever, just the fat transfer to the brow, lower eyelid, anterior cheek, outer cheek, transition into the lower cheek, and jawline (although I can't see the jawline 100% because of the beard).
As far as questions you raised in your email, here are my responses:
1. Smoking is not ideal, as you know. However, I have had success with smokers' fat grafting taking and holding. I just make no promises. Ideally, you consider stopping 2 weeks before and 2 weeks after but that may not be realistic. I am still very confident that you will be fine.
2. As far as after care, there really is very little to do so if you have questions that remain for me after you leave, you can always email me or call me. I am readily available for my patients so you should never feel alone.
