yes, i have a strong viewpoint on this. i do not use fat to fill a small defect for many reasons:
1. it is metabolically active and acts like body fat so you can't just fill a little in there because it could change with body weight and become uneven. that is why i use fat very carefully and blend in the various areas of the face.
2. if the take in that one tiny area is poor, then all the work of sedation, harvesting, processing, centrifugation, injection leads to nada, that makes no sense. that is why i talk about the 5% rule meaning that a good fat transfer is about total face blending so that if any one 5% area does not look as good as possible the face still looks amazing.
3. fat, as you mentioned and as i have mentioned, is too soft to make a difference in a little area. too unpredictable in both directions, too little or too much.
if it is very small like a dime size and you want a permanent correction, i would most likely treat it like an acne scar and use silicone micro injections.
