Generally, the fat holds pretty well in the chin. As we age, we lose both bone and fat. If you were born already with a small chin and especially if you are a male, a chin implant is better and more predictable for what is called projection (the degree to which the chin sticks out). However, if you are aging and especially female, then projection is oftentimes not as critical (unless you really have a small chin) then fat grafting can cover a multitude of sins even for a slightly underprojected chin. Where fat fails is completely getting rid of a jowl or even managing fine lines or puppet lines around the mouth. It can be done but the success rate for doing so in my opinion is poor.
