this is not easy to answer because it depends on your specific issue. i personally do not like using kenalog, as i believe it is both ineffective and risky. typically lower eyelid fat excess must be excised. if it is medial (toward the nose), i tend to use an incision along the tear trough if you have a visible one that tends to hide the incision pretty well. again, you need a personal consultation with a physician to get a better answer but answers will differ as this is both a difficult situation and depends on the surgeon's preference of approach.
